When you have all the required information listed below, please email us to arrange a reservation.

Required Information for Reservation

(1) Patient’s full name in alphabet (if applicable, in Kanji Character or Katakana),
　　Date of Birth, Age and Gender.
(2)　Letter of recommendation with diagnosis from your previous doctor.
(3)　Contact information (name, e-mail or daytime phone number).
(4)　CD Rom which contains most recent results of CT or MRI.
　　※CD-Rom should to be formatted with DICOMDIR System, uncompressed.
　　　Please send it by postal mail.

• By Train:
  Take JR Joban Line/Local (=Metro Chiyoda Line). Get off at “Kameari Station”, exit at North side. Walk approx. 15 minutes.


• By Adachi Community Bus:
  Take “Harukaze” Line. Get off at “Towa Hospital” from either Kameari station (5 minutes) or Ayase station (20 minutes).


• By Taxi:
  5 minutes from Kameari Station, 10 minutes from Ayase Station.
